People leaving hospital

If you are leaving hospital and you do not have a home or have a home that has become unsuitable, then you should tell ward staff as soon as possible.

With your consent, the hospital can refer you to a local housing authority under a 'duty to refer'. You may receive more help if you are referred to a local housing authority in an area to which you have a local connection.

The local housing authority will contact you to find out more about your situation before deciding how you can be helped.
